I have organised the fields on the form into four sections: general, your television viewing, your computer use and other activities. I think it makes sense to group questions about the same topic together. I have used drop-down menus to cut down on the amount of typing users have to do and make sure they choose one of the allowed responses.

I made a recording of my friend Charlotte using the data entry form to enter her details into the database. She said that the form was easy to use. She used the Tab key to move from field to field and the mouse key to select from drop-down menus.

I also asked my sister Amy to fill in the form. She needed help to understand what some of the questions meant because she is not familiar with the database
